Texas Health Harris Methodist Hospital Southwest Fort Worth has an immediate opening for a Social Worker-BSW/LSW. This position is PRN (as needed)

LBSW. MSW a plus.

Minimum Requirements - EXPERIENCE:

6 months experience in a social services setting preferably in a hospital or other health care institution.

Minimum Requirements - SKILLS:

Must be able to use Microsoft office products.

• Identify cases for social work management involvement by diagnosis, procedure, high utilization, or high costs. Screen all patients
• Protect hospital from risk management issues.
• Use knowledge of healthcare delivery, anatomy, and physiology to facilitate quality care.
• Identify healthcare ethical issues and work to address them.
• Comply with regulatory standards (TJC, EMTALA, HIPAA, TDI, ADA, CMS).
• Work to reduce Avoidable Days and Denied Days.
• Help patients and families with governmental healthcare eligibility and benefits.
• Assess for appropriate level of care (Acute, LTAC, Rehab, SNU, NHP, HH/DME, Hospice, and Psychiatric).
• Complete and document psychosocial screening and assessment in a timely manner as defined by Department policies, guidelines and patient needs.
• Formulate and implement effective discharge plans.
• Work effectively with the care coordinator and within an interdisciplinary team model.
• Proactive approaches to discharge planning
• Demonstrate a breadth and depth knowledge of community resources.
• Address issues related to patient’s non-compliant behaviors.
• Refer patients/families to emotional support resources.
• Reduce/eliminate barriers to accessing healthcare and promoting health recovery.
• Develop and maintain knowledge of current legislative changes affecting healthcare practice.
• Address patients future care needs such as SNF or Nursing home care or returning to the work force, when needed.
• Collaborate with other healthcare professionals to provide health wellness and illness/injury education.
• Explore patient/family’s current and projected financial resources for healthcare and essential needs (employment, SSDI, SSI, Work Comp, STD, LTD, FLMA, CBA).
• Make referrals to special entitlement staff as indicated.
• Act on behalf of vulnerable individuals and take appropriate actions to safeguard their interests and rights (child maltreatment, elderly maltreatment, disabled maltreatment, domestic violence, incapacitated/incompetent).
Explain services and how to access resources in understandable terms.
• Obtain healthcare related authorizations as needed and/or communicate to ensure authorization is done by the accepting vendor/agency (ambulance transport, transfer to the next level of care).
• Seek opportunities to reduce inappropriate utilization while maintaining quality care.
• Help patients/families navigate the complex healthcare system.
• Synthesize other discipline’s findings to create a multidisciplinary plan of care.
• Plan and coordinate related aspects of transfer of services across the continuum of care.
• Ensure continuity of care within limitations of confidentiality.
• Perform Cost/Benefit Analysis of continuum of care and alternative care options.
• Establish and maintain effective, credible relationships with attending/consulting physicians.
Anticipate physicians practice patterns, document verbal orders and/or request for doctor’s order to meet patient discharge needs. Conference with physicians as needed to expedite patient’s progress through continuum of care.
Work closely with care coordination so that the patient is ready for discharge at the tiem of discharge
