RN Staff Hospice Inpatient - FT/PT/PRN; ALL SHIFTS AVALIABLE

Job Summary:

The RN identifies patient needs and priorities for care though assessment. The RN plans and delegates care based on patient needs. The RN evaluates the patient's response to care and the attainment of or progress toward expected outcomes. The RN is accountable for outcomes of patient care delivered during the assigned shift.

Minimum Knowledge, Skills, Experience Required:
Must hold and maintain a current Alabama Nurse License.
Must maintain a current Alabama Driver's License and automobile liability insurance.
Experience in nursing of at least one year.
Must be able to lift, position, and/or transfer patients.
Must be able to lift supplies and equipment.
Duties also require prolonged or considerable walking or standing, reaching, stooping, bending, kneeling, and/or crouching.
Must have visual acuity and hearing to perform required nursing skills.

Key Responsibilities/Essential Functions:
Responsible for making nursing visits as assigned by Clinical Manager, updating Plan of Care as needed, responsible for accurate and timely completion of all documentation in EMR.
All documentation from patient encounters will be documented at time of visit. "Time of visit" is defind as time at bedside, time in facility, or time in car immediately following visit. Exception to this will be granted by direct supervisor.
Communicate with physicians, team and caregivers to evaluate and make changes as appropriate to provide pain and symptom control. Educate patients and families regarding pain and symptom management medication, death and dying, etc.
Educate patients and caregivers regarding Hospice philosophy/mission and all services available under HFC services.
Educate and empower family/caregiver to provide care for the hospice patient.
Provide support to patients and caregivers in all domains: physical, emotional, spiritual and psychosocial.
Assist patients and caregivers to cope with day to day activities and emergencies that may arise while under HFC services.
Act as a liaison between the patient/caregivers and the physician. Act as an advocate for the patient and caregivers.
Demonstrate knowledge of Medicare, Medicaid, NHPCO, and HFC criteria guidelines for admissions, recertifications, revocations and discharges and document within these guidelines.
Responsible to conduct Hospice Aide (HA) supervisory visits every 14 days (if applicable), to document visits on the proper form, and to evaluate the care provided by the HA with a written report of any problems.
Communicate with team members to provide information and updates regarding patient and caregivers status and to receive pertinent information regarding the same.
Attend educational/training opportunities.
All hospice registered nurses are expected to participate in clinical on-call as needed.
IDG. Demonstrate knowledge of the patient's Medicare benefit periods and accurate assessment of the patient's status.
Participate in bereavement follow-up as assigned by Clinical Manager.
Maintain confidentiality.
Perform other duties as assigned.
