Pool Position Day Shift
Position Summary:

Under the general direction of the rehabilitation director, provides physical therapy services to patients following the plan of care established by a physical therapist.
Essential Functions:

Essential Functions:

Under the supervision of a physical therapist, provides physical therapy services, reassessing the patient's status on each visit and ensuring quality of care as evidenced in the clinical record.

Implements the plan of care (POC) as established by the physical therapist.
Instructs patient and caregiver in treatment programs, precautions and equipment use as necessary and documents patient/caregiver demonstration of instructions provided.
Maintains frequency of visits as established on physician's order or POC.
Documents patient status and progression towards achievement of outcomes on each visit in measurable parameters.
Able to ask appropriate questions of patient/caregiver and collect data necessary to monitor patient's status.
Modifies interventions in accordance with patient changes, within the scope of the POC.
Reports changes in patient's status and updates on progression of POC to the supervising physical therapist or rehabilitation director.
Collaborates with physical therapist and patient in ordering equipment necessary for patient safety and optimum function.
Demonstrates safe techniques in providing patient care, as observed by the director and other care providers.
Coordinates patient care with other clinical team members ensuring optimal patient care and communication as evidenced in the clinical record.

Makes recommendations to and participates in the revisions to the plan of care.
Utilizes appropriate community resources.
Recommends and participates in case conferences.
Updates other team members of changes in patient status that will impact on the plan of care.
Identifies the need for and makes recommendations for referrals to other disciplines.
Maintains and enhances personal knowledge as well as promoting professional standards in physical therapy.

Maintains current license and submits timely evidence of renewed license.
Completes all required documentation following treatment session to ensure that documentation reflects care provided and the outcomes achieved, as evidenced in the clinical record.
Demonstrate Standards of Behavior and adhere to the Code of Conduct in all aspects of job performance at all times.
Qualifications:

Required:

Graduate of an approved associate degree program as a COTA
Currently licensed in Washington State
One year experience in a hospital or rehabilitation facility preferred
NBCOT certified OTA
Must be currently certified in BLS and recertified every 2 years.
Must possess basic computer skills related to Windows navigation, mouse usage, accurate keyboarding, email communication and password management
