PRN Rotating weekends/holiday Days Position Summary: (Provide a BRIEF OVERVIEW and overall Function this position provides; should be inspirational)

· PTA's are entrusted to use and develop their education, training, and experience with the intent of collaborating with the patient and care team to create an environment driven by the needs of the patient and their family and to facilitate each patient's progress toward independence with living, working, and playing in a healthy and safe environment.

Required Qualifications: (MUST HAVE: MINIMUM knowledge, technical skill, proficiency, training, license, education, experience, etc)

· Current NM License to practice as a Physical Therapist Assistant.
· Current CPR certification.
· Current PPD screening.
· Fluent in spoken and written English.
· Meet physical requirements.
· FIM training and passing of the FIM certification test within 6 months of employment.
· Proactive with patient safety (fall reduction, infection control, hand hygiene, patient identifiers).
· Basic computer skills for documentation and printing necessary documents.


Preferred Qualifications: (MAY HAVE: 'higher' education, leadership qualities, 'special skills, etc )

· Organizational skills strong enough to implement the patient care process.
· Communication skills strong enough to participate with the team, manage conflict, and work successfully with the public
· If Navajo or Spanish speaking ? complete interpreters course and pass test annually.


Duties and Responsibilities: (SPECIFIC summary of the work that will be performed, including end results and key means by which the end results are achieved. Give an idea of the general nature, purpose and levels of skill and responsibility found in the position)

· Receive patients for treatment under the direction of the physical therapist.
· Perform daily treatments and re-assessments within the professional scope of practice commensurate with education and training
· Communicate with and assist the physical therapist in revising the treatment plan and therapy goals.
· Provide patient and family education relevant to their needs, equipment recommendations and use.
· Complete timely documentation including weekly progress and conference notes, daily treatment notes, pt/family education, discharge/transfer/referral forms and other required documentation, assist the physical therapist with completion of the discharge summary.
· Complete timely and accurate FIM scoring.
· Collaborate with the patient, family, and all other members of the rehabilitation team.
· Delegate and instruct support staff regarding the services they may perform.
· Promote patient rights, confidentiality, privacy, and dignity.
· Provide education and training to other staff members, informally or formally to meet the needs of patients, families, and staff.
· Participate in and support Performance Improvement activities.
· Attend team and facility meetings.
· Demonstrate professional growth and development by attending continuing education programs and self-study.
· Serve on at least one facility committee or task force.
· Implement SJRRH's Service Standards into daily work.

Physical Demands:
o Heavy physical demands may be placed upon the employee. The amount of time that the employee is exposed to these demands will fluctuate due to uncontrollable variables in healthcare ? ex. Patient's height and weight, level of cooperation and physical dependence, specific clinical needs, etc.
o Job performance may require lifting, pushing, and/or pulling various weights, possibly in excess of 200#, with mechanical and other staff assistance available at times.
o Activities may include intermittent or prolonged standing, walking, sitting, talking, hearing, using the hands to manipulate/stabilize/feel, pushing, pulling, stooping, kneeling, crouching, crawling, reaching with arms, and smelling.
o Repetitive motions may be required, including wrist and hand movements, grasping, finger dexterity such as in typing, writing, filing, etc.

Sensory Demands:
o Employee will need to be able to hear alarms on equipment, fire alarms, overhead announcements, conversation including patients, families, and other healthcare providers
o Vision: Near and distant acuity, color discrimination, depth perception, peripheral vision.
o Smell.

Work Environment:
o May include exposure to the following environmental indoor conditions: fumes or airborne particles, risk of electrical shock, risk of vibration, fluctuations in indoor temperature.
o Noise level from very quiet to very loud.
o Potential for exposure to infectious patients and materials.

San Juan Regional Medical Center is a 240 bed acute care hospital with a level III trauma center located in Farmington, New Mexico. In August 2006, an $83 million expansion was completed which added 72 new private patient rooms, most with fresh air balconies. The expansion also added 8 new operating suites, all with ceiling-mounted boom technology, a new public cafe, a healing garden, a well equipped staff/physician exercise room, and a new Central Sterile Processing facility. The older part of the facility is undergoing renovation, with all patient rooms being converted to private. Much of the renovation is completed, including the new staff-only cafe, and a spacious Cardiac and Pulmonary Gym.
