The Nurse Care Manager is a registered nurse with an advanced level of pediatric clinical experience. Demonstrates thorough knowledge of managed care, utilization management, discharge planning, and performance improvement concepts. The Care Manager is a self-motivated health care professional capable of performing complex care coordination planning for patients with serious, life-threatening illnesses of different ages and developmental levels focusing on those patients with complexities related to clinical, finance and/or resource utilization components. The Care Manager will advocate for the patient/family by collaborating with the multidisciplinary Palliative Care (PC) team to develop and implement a care plan to ensure quality patient outcomes, and customer satisfaction in a cost effective manner. The Care Manager will participate/facilitate in the development and outcome analysis of clinical pathways as a member of the multidisciplinary team. Demonstrates commitment to a patient and family focused model of care.

Minimum Education/Training Required:
Bachelors degree in Nursing required. Masters degree in Nursing, Health Care Administration, or related field preferred.

Minimum Experience Required:
Three years, nursing experience with two years pediatric experience in Palliative Care, one year of utilization management and/or discharge planning experience required. License, Registration or Certification Required: Current Registered Nurse licensure in the State of California. Current AHA CPR training.

Skills Required:
Fluency in both English and Spanish desired. Computer Literacy Well developed written and verbal communication Skills. Strong interpersonal and negotiation skills.
