The Child Advocacy Care Coordinator is responsible for the application of extensive knowledge in clinical performance improvement activities. This individual will be responsible for driving engagement strategies for improving physician and staff patient care practices based on the Child Advocacy care clinical guidelines, best practices, and internal and external benchmarks. The child advocacy care coordinator supports and ensures the overall success of the community health model by collaboratively working with patients, physicians, practice teams, and health plans to select, guide and direct patient care, while integrating key features of the medical home. The Child Advocacy Care Coordinator will work with a multidisciplinary healthcare team in a primary care setting. The Child Advocacy Care Coordinator will work alongside the physicians and office staff in a community health, focusing on identifying the needs of complex patients while assisting the practice to develop a process for managing its patient population. This position will join physicians, social workers, a nurse practitioner, a clinic manager, and an office coordinator in Child Advocacy at Dayton Children's, a pediatric subspecialty dedicated to the evaluation of children/adolescents for suspected abuse and neglect. Confidentiality, sensitivity, and compassion are paramount to the trauma-informed care provided by this team. This position offers clinical work and care coordination in a busy and diverse multi-site outpatient practice, as well as opportunities for education and community outreach, research, and quality improvement. The team is actively planning for continued expansion.



REQUIRED: BSN, 2-5 years experience, RN licensure in the state of Ohio, Excellent oral and written communication skills- be able to provide information in a clear and concise manner, good interpersonal skills. Self- motivated, and have ability to prioritize work, decision-making and problem-solving skills. Basic computer skills



Preferred: 5-7 years experience specializing in pediatrics, Strong knowledge of data analysis, Quality Improvement, and experience with project management in a healthcare environment. Word, Excel, Powerpoint.
