Physical Therapist-Acute and or Outpatient

Required Qualifications:

· NM State License as a Physical Therapist
· Sufficient previous experience to perform duties
· Fluent in English
· Physical demands ? refer to "Rehab Dept Physical Demands"
Duties and Responsibilities:

· Interpretation of referrals & orders to check for appropriateness, indications and contraindications
· Initial evaluations/ Discharge summaries/ Re-evaluations/ Daily treatments:
· Are performed & documented in timely manner , may include but are not limited to:
o Medical diagnosis, medical history, objective physical/cognitive findings, measurable / functional/ behavioral based patient goals, treatment plan, discharge plan, education and communication as indicated.
· Determination of which tasks require the expertise and decision making capacity of the P.T. and must be personally rendered by the P.T. and which tasks may be delegated.
· Delegation and instruction of the services to be rendered by the supportive personnel, including but not limited to specific treatment program with patient goals / plan, precautions, special problems or contraindicated procedures..
· Education and instruction as indicated, including, but not limited to: patient, family, other staff, follow up facilities.
· Transportation of patients as needed.
· Maintain, gather, clean ,set up all equipment per facility policies
· Daily record Keeping, including (but not limited to) evaluations, daily treatment notes, discharge notes and daily charges, all communication/education provided (per facility / dept policy)
· Each employee is responsible for implementing SJRMC's Service Standards into their daily work.

Additional Requirments / Information

San Juan Regional Medical Center is a 240 bed acute care hospital with a level III trauma center located in Farmington, New Mexico. In August 2006, an $83 million expansion was completed which added 72 new private patient rooms, most with fresh air balconies. The expansion also added 8 new operating suites, all with ceiling-mounted boom technology, a new public cafe, a healing garden, a well equipped staff/physician exercise room, and a new Central Sterile Processing facility. The older part of the facility is undergoing renovation, with all patient rooms being converted to private. Much of the renovation is completed, including the new staff-only cafe, and a spacious Cardiac and Pulmonary Gym.
