Home Care Coordinator

Day Shift CPR Required Licensure Required two or more relevant experience The Home Care Coordinator is a licensed nurse who has a thorough understanding of home care products and services and the services and resources available in the community. The Home Care Coordinator act as a Home Care resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient referred to receive home care services. The Coordinator excels in skills of interviewing, and diplomacy and has a broad knowledge of community resources.

Education: Graduate of accredited RN/LPN program required. B.S. in Nursing preferred.

Knowledge: Knowledge of Medicare Conditions of Participation. Thorough knowledge of community resources including methods for reimbursement. Thorough understanding of agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services.

Skills: Excellent organizational and communication skills. Flexibility in dealing with a variety of personnel. Ability to interpret agency philosophy and policy. Ability to communicate clearly and positively to insurance companies and all levels of staff including physicians, nurses, social workers, case managers, discharge planning staff, patients and family members. Ability to identify patients' needs for home care, to plan for the continuity of care and communicate that plan effectively and efficiently. Ability to be self-directed and confident working in relatively unstructured situations. Ability to travel between multiple inpatient/outpatient hospital settings.

Required Licensure, Certification, Etc.: RN/LPN in the State of Maryland and CPR.

Work Experience: Two (2) years home care or home care discharge planning experience. Demonstrated excellence in nursing assessment, home care planning and problem-solving skills. Demonstrated clinical expertise in the clinical area of assignment; i.e. oncology, infusion therapy, pediatrics.
